Connect to first hdmi port (closest to power connection) and boot with just the display connected (no sdcard).
What does the "DISPLAY:" line report in the diagnostic screen?
Next, boot normally, and run "raspinfo | pastebinit" (e.g. connecting through ssh, and report the url returned).
Do this once when display is working as expected and once when it is not working. We'll see what the difference is.
That machine is now running my NAS, so that would currently be unduly disruptive, but the next time i am tinkering with it i hope to have the presence of mind to recall this post and try that out.
@jamesh - thank you for the PDF link. A cursory glance reveals this snippet on page 5:

That's what happens roughly half of the time, and i rather assume that it's a minor hardware glitch. The machine otherwise functions 100% perfectly. The doc offers config options which look promising, which i'll try the next time that machine is taken offline for maintenance.
