sorry, can't offer any further support here as I've never experienced this.I can’t get to that pi now, but I can look into anything you need and reply later.
The fan works fine at all temperatures, and is mostly off, but it starts spinning at top speed when the pi is shut down, and stays that way till I disconnect the power supply.
I had it configured before ( bullseye?) so that the fan would be OFF on shutdown. I just realized that such fix isn't working anymore.
How do you shut down your Pi? Maybe that procedure causes issues with GPIO18.
